Severe drop in blood pressure on standing: What causes this?


Question.


What causes a severe drop in blood pressure when a person stands up from a seated position?



Answer.

A severe drop in blood pressure when you stand up which can lead to dizziness and fainting has many possible causes.

When you stand up, the effects of gravity normally cause a slight decline in your blood pressure (orthostatic hypotension). Blood naturally wants to pool in your leg veins when you change positions from sitting or lying down to standing. Typically, your autonomic nervous system makes up for this effect by narrowing your blood vessels and increasing your heart rate. This helps maintain blood pressure by increasing the efficiency with which blood returns to your heart.

--Some possible reasons why these adjustments may fail to prevent a drop in blood pressure include:

-Overly aggressive treatment with blood pressure lowering drugs
-Certain medications, such as psychotropics and sildenafil (Viagra)
-Severe dehydration
-Abnormally slow or fast heart rhythm (arrhythmia)
-Prolonged bed rest
-Disorders of the autonomic nervous system, such as diabetic neuropathy


Treatment of severe orthostatic hypotension depends on the underlying cause but may include:

-Support stockings
-Avoiding triggers such as dehydration and certain medications
-Medication to increase blood pressure
